The goal of Life Care Planning is to promote and maintain the good health, safety, well-being, and quality of life of elders and their families.

We provide assistance with the legal, financial and care-related challenges elders face.

with our knowledge and expertise.

Through Life Care Planning, elders and their families get access to a wider variety of options for care, as well as knowledgeable guidance from a team of compassionate advisors, who help them make the right choices about every aspect of their loved one's well-being - including finding available resources and benefits to pay for quality care.

Our on-staff Elder Care Coordinator offers support to families whose elderly loved ones are at home, in assisted living or skilled nursing facilities. She will coordinate needed care, community services, and serve as the elder’s advocate.  This lightens the load for family caregivers, improves the elder’s qualify of life, and can even postpone the need for nursing home care.
